How extensive is the scarring after the tummy tuck?There are quite a number of people who are afraid of going through a tummy tuck even if qualified. The fear stems from reasonable concerns about post-operative scarring. While the tummy tuck is a major plastic surgery, and the scarring is relatively deeper and extensive, there are many factors in play that determine the extent of scarring after a tummy tuck.

There are different incision placement techniques for a tummy tuck surgery and if properly placed, your scar may not be as extensive or as visible.

Numbness Of The Abdomen After A Tummy Tuck

 

If you have a tummy tuck, you will notice that your abdomen feels numb after the procedure. For those who are still considering a tummy tuck, then you need to understand that your abdomen will be numb and will take months to recover.

You may wonder why a tummy tuck makes your abdomen numb. The reason for this is during a tummy tuck procedure, the surgeon has to make a cut in the lower abdomen and then lift the skin from the abdomen. When the skin is lifted, it essentially causes the nerves to separate. This causes the numbness because if you look at the anatomy of the abdomen, the nerves that supply sensation to the abdomen comes from the top and also through the muscles of the abdomen, and then they go up towards the skin. So when the surgeon makes the incision, these nerves that supply sensation to the abdomen, particularly in the lower half, are damaged. This is inevitable because the nerves are too small for the surgeon to see and avoid slicing through. Thus, sensation of the abdomen will be compromised. Consequently, you have to be careful after a tummy tuck because you may not even realize if you are injured in that area.

Pregnancy not only loosens the skin, but it also causes excess skin in the abdomen. It can be severe enough to require not just a tummy tuck, but also the surrounding aesthetic areas of the abdomen. One of the areas that is greatly affected is the mons pubis. This is the area above the vagina. Most of the time, pregnancy causes it to sag and drop. Fortunately, this can be resolved through a tummy tuck.

 

During the tummy tuck, the excess skin in the abdomen and the mons area will be addressed. Before making an incision, I need to mark the area of the abdomen I will be slicing. While I do this, I pull the mons and the vagina upwards to ensure that the scar is in the right position. This is important because if you don’t make the incision in the right location, then the excess skin will remain, leaving sagginess in the vagina area with unsatisfactory results.

The key is to make the incision as low as possible, but at the same time being able to pull the excess skin from the mons pubis area. In other words, the skin is pulled mainly from the mons to lift it, utilizing the elasticity of the abdomen. The surgeon must determine the location of the scar by assessing the excess skin on the mons before the procedure. If we do not do this, then your mons is not going to have the aesthetic result that you are looking for.

It seems counterintuitive to discuss soft tissue relaxation after a tummy tuck. The main reason to have a tummy tuck is to have a tighter belly. But how tight your belly is going to look after surgery depends on your skin tone and elasticity prior to surgery. We have discussed this in another video. But you need to understand that in some particular patients, soft tissue relaxation means the skin of your tummy gets slightly looser after the procedure. Typically patients that have these issues are patients that have poor skin elasticity. For example, the patients that have stretch marks above the bellybutton are a higher risk of having this issue.

 

The patients that have stretch mark just below the button most of the time, they do not develop this problem. The reason is that any skin that still has stretch marks is weak and when tension is applied to the skin, it slowly and steadily relaxes, creating small areas of soft tissue relaxantion after the surgery.

Setting the tension before surgery is extremely important, because the tighter the skin is made, particularly in those patient that will have stretch marks above the belly button, the less likely these occur.

The problem is that the more skin you cut, the higher the horizontal scar your tummy tuck result will have. In other words, the more skin is removed, the higher your horizontal scar might end up.

So what happens if you develop a slightly looseness of the tummy after surgery? Well, in essence you are going to need to have a revision surgery where some of the excess skin will be removed.

Understanding skin tightness and relaxation after surgery is important for you to have realistic expectations on how your belly is going to look after surgery.

If you develop soft tissue relaxation, it does not mean that the surgeon did not cut enough skin. It means that your skin tone is poor in some cases and relaxation of the skin is just a normal process of the healing after a tummy tuck.

Tummy Tuck

Every single patient that comes to the clinic is extremely concerned about the length of the scar after a tummy tuck. As a matter of fact, one of the first things that they ask for when we start a conversation is a mini tummy tuck, rather than a traditional tummy tuck. Usually, the scar of the tummy tuck goes from hip to hip, which is the standard when it comes to tummy tuck scars. The reason is that most people’s skin has been stretched significantly in all quadrants of the abdomen (i.e. upper and lower quadrant on the right and the left side of the abdomen). So if you have excess skin everywhere on your abdomen, there is no way the surgeon can cut the excess skin with just a small incision like in a mini tummy tuck.

 

The mini tummy tuck is a procedure that results in a much smaller scar than a regular tummy tuck. However, the surgeon cannot do a mini tummy tuck on everybody. This will depend on the your skin. The more excess skin you have, the longer the scar.

For the traditional tummy tuck, you will have a scar from hip to hip. In some patients, we even have to extend it farther than the hip, and sometimes all the way to the lower back in order to close the incision to avoid producing dog ears. If you try to cut corners because you want a shorter scar, the excess skin around the scar will bunch up on the sides, creating what is called a dog-ear. This is a very unsatisfactory and unappealing result of a tummy tuck, which any surgeon wishes to avoid at all costs.

To understand what determines the length of your scar, it requires a bit of math and geometry. If we look at the abdomen from the bellybutton and we draw a line to the sides where the skin needs to be removed, once we connect this to the lower abdomen, the shape that it creates is like a triangle when we put all this together. So when we close, the two lines on the top need to match the line on the bottom part. In other words, the amount of excess skin needs to match the length of the triangle that we are creating.

Your surgeon may boost the size of your bottom from the side with fat grafting; however, he or she may not necessarily be able to reshape your body into an hourglass. Only an experienced, innovative, and skilled plastic surgeon can do this perfectly.

For this reason, whenever you have a tummy tuck, one of the things that needs to be determined prior to surgery is more or less how long the scar is going to be. If you are not willing to accept the length of the scar required to remove all your excess skin, then you are not going to be a satisfied customer. Therefore, you should take into account that the length of the scar is crucial to having impressive results, but at the same time will leave scarring. It is up to you to decide whether you can live with that, so make sure to consider this before the surgery.

Location of the tummy tuck scarBefore choosing to do a tummy tuck, one of the main concerns of patients is how big the scar will be and where it will be located. When it comes to the location of the tummy tuck scar, the surgeon marks the scar as low as possible so that it is less noticeable and you can even cover the scar in a bikini.
However, even though the scar is meant to be low, some patients may have it lower than others. This is determined by the following factors:

 

  1. The skin condition and amount of excess skin
  2. The distance of the belly button to the lower portion of your abdomen where the scar is going to be located
  3. If your skin is very loose that means that your skin can stretch more. The more your skin is able to stretch, the lower the location of the tummy tuck scar is going to be. When we close the incision on the abdomen, the top part of the skin and the lower part of the skin are brought together. You will notice there are two vectors in your scar, one going down and one going up. Because of the skin’s elasticity, the scar will lift to some extent when it feels the tension in the closure of the incision. In other words, if your skin is more elastic then there is more skin from the top, making it possible to position the tummy tuck scar lower.

On the other hand, if you have a very tight skin then the skin is going to resist being stretched. When this happens, the closure will make your tummy tuck scar higher.

Now another thing that you need to consider is the distance between your bellybutton and the lower part of your abdomen. The longer the distance is, the higher your scar will be. The reason for this is because as I mentioned in previous videos, the tummy tuck involves cutting the skin below the bellybutton. The longer the distance between the point where we have to suture down from the abdomen to the lower part of the abdomen, the more tension there is going to be. The tension causes your tummy tuck scar to rise.

Therefore, you will certainly have scarring after a tummy tuck. It is only a matter of how high or low will the scar be, which depends on the two factors aforementioned. Fortunately, you will be able to hide the tummy tuck scar with your bikini or underwear after surgery.
